In the prevention plan I would make sure to include all this information along with short term goals. Long term goals are good, but they can become over whelming to look at and can lend to discouragement research has shown that it is better to break the long-term goals into multiple short-term goals that are more compensating to reach. of coarse you would make sure to have all their personal information on the prevention plan like name, residence and contact information. I think documenting any resources that were set up or provided to the client should be on the prevention plan. Also, a list of future scheduled visits / or meetings that should be attended.
